Jay Hazen, DDS

Jay Hazen DDS

Dr. Jay Hazen is a 1991 graduate of the Marquette University Dental School. He has several hundred hours of continuing education credits in all phases of dentistry including intensive training and education in aesthetic and reconstructive dentistry and is a current member of the following:

  • Graduate of LVI - Las Vegas Institute of Advanced Dental Studies
  • American Association of Cosmetic Dentistry
  • Academy of Computerized Dentistry
  • Academy of General Dentistry
  • American Dental Association
  • Wisconsin Dental Association
  • Dr. Hazen has been named one of the top cosmetic dentsits in Madison by his peers

Sarah Karls, DDS

Sarah Karls DDS

Dr. Karls earned her B.S. in Biology from the University of Wisconsin-Milwaukee and D.D.S. from Marquette University School of Dentistry. After dental school she completed a General Practice Residency at MeriterHospital, where she studied advanced techniques in dentistry and treated the underserved and developmentally disabled in a hospital setting. Dr. Karls is currently a faculty member at MeriterHospital and helps provide instruction and guidance to dental residents. She also volunteers at the Madison Dental Initiative, offering dental services to low income and homeless individuals. In her free time, she enjoys golfing, yoga, and spending time with her family.

Jeni – Dental Assistant

I have been a dental assistant since 1996, previously a surgical tech. I am committed to making your dental experience as comfortable and easy as possible. Dr. Hazen and Dr. Karls have the tools, techniques, and advanced training to provide exceptional care to our patients.

Trisha – Dental Hygienist

I have been a part of the Dentistry For Madison team since graduating from the MATC program in 1996. I love that the world of dentistry is always changing and improving and our practice is changing and improving along with it. I think that our patients are impressed with how new technology is in our practice and it makes it much more enjoyable. I like being a dental hygienist because it is not only about teeth, but also about the relationships formed with our patients. I also think educating our patients is key. Patients are amazed at how interesting their mouths are!

Diane – Dental Hygienist

I am a graduate of the dental hygiene program from MATC and have been practicing at Dentistry For Madison since 1996. What keeps my job exciting is the technology and treatment that we have available to our patients. Our office is committed to offering the best possible treatment in an atmosphere that reassures the patient that he/she is getting the best care possible.

Angie – Dental Hygienist

I graduated from the MATC Dental Hygiene Program in 1999. In 2004, I jumped at the opportunity to become part of the Dentistry For Madison team, and haven’t looked back since. I love what I do, and love our patients. Our staff has truly become a family over the years. We work well together, and are committed to offering the best service possible. Quality care, with the latest technology, in a comfortable environment is what we are all about.

Mary – Dental Hygienist

Dentistry is a family affair. I have twin brothers who are orthodontists and a sister-in-law who is a hygienist. With dentistry as a family trait, I started my career as a dental lab technician, and worked my way into dental assisting. Realizing I wanted to be more involved with my patients prompted me to go back to school to become a dental hygienist. Being intrigued with the upbeat atmosphere and advanced technology at Dentistry For Madison, I started working with them in April 2007. I enjoy being a team member and love the relationships I have made with our patients. Being a hygienist, along with my Natural Health bachelor studies is very rewarding. It enables me to help people with their great smiles and overall health. My interests include sports, music, fishing, camping, boating and cycling.

Brenda – Insurance/Treatment Coordinator

As a member of the Dentistry For Madison team, I assist you in the coordination of treatment, insurance and financial arrangements. I have been part of the team since 1989, in both the front office and clinical capacities. My goal is to help provide the very best dental care to our patients, so that each patient achieves optimal dental health throughout their lifetime. Quality, comfort and state-of-the-art dentistry is what Dentistry For Madison promises and delivers!

Roxanne – Scheduling Coordinator

I have been a Dentistry For Madison teammate since March 1990. My goals for you are that we make you feel comfortable as you become a part of our practice. We hope to fulfill all of your dental needs with our commitment to your dental health.

Diann – Office Manager

As a member of the team since 1995, my goal is to make your visit to our office as enjoyable as possible. We take pride in the fact that we know our patients - from their fears and worries to the latest news in their life. It is very rewarding to educate and help people, whether it is getting them out of pain or improving their smile. It is true “a smile can change a life,” we see it every day.
